  I was going to enter, but my state (Hawaii) is not on the roll down list of states. What's up with that?

Hey Birdmove!  8)

It specifically names Puerto Rico and other territories, but doesn't disallow our favorite island state specifically.
I'd try entry method #2:

Doesn't have Canada, either, but I filled it in (with province and Canada in with the city)....
